Application for Family Allowance

This form allows you to apply for Family Allowance in the following situations:

  • Birth and adoption: You adopt a child or your child is born outside Québec but lives with you in Québec.
  • Separation, change in custody or death: You are not receiving Family Allowance payments in your name and you have at least 40% of the custody time of the child following a separation, a change in custody or in the event of the death of a parent.
  • New resident: You become a resident of Québec or your child returns to Québec.

It is also possible that Retraite Québec asks you to complete an application for Family Allowance for another situation.

If your child was born in Québec, you do not need to file an application because registration is carried out automatically. When you send your newborn's birth certificate to the Directeur de l'état civil, you are automatically signed up to receive Family Allowance.

Change in Frequency of Family Allowance Payments

This form allows you to change the frequency in Family Allowance payments. Family Allowance payments are usually made 4 times a year (in July, October, January and April), but you could choose to receive monthly payments. You will be able to revert to quarterly payments as needed.

Change in Conjugal Status

This form allows you to inform us of any changes to your conjugal status.

The parent who is currently receiving Family Allowance payments in his or her name must use this form to inform us of a change in his or her conjugal status. In the case of a separation, you must be separated from your spouse for at least 90 days before notifying us.

The parent who is not receiving Family Allowance in his or her name must file an application for Family Allowance if the child lives with him or her at least 40% of the time each month.

If the change in conjugal status also involves a change in child custody, you must inform us immediately by telephone.

Direct Deposit of Family Allowance Payments

Use this online service to sign up for direct deposit of Family Allowance payments or make a change to your banking information. The application can only be used for deposits in Canada.

Change of address

Foreign Income (new resident of Québec)

New residents of Québec must use this form to report any income earned outside Québec or Canada for the years required in order to calculate the amount of the Family Allowance.

Immigrant Status in Canada

A beneficiary can use this form to declare to Retraite Québec his or her Canadian immigrant status or that of his or her spouse if neither one is a Canadian citizen.

Change in Percentage of Custody Time

This form is for former spouses who share the Family Allowance payments and who wish to inform Retraite Québec that the percentage of custody time per month has changed.

Change in the Number of Dependent Children

This form must be filled out and sent to Retraite Québec if a parent no longer has custody of a child following a change in custody or if the child has left the family.

Statement of Foreign Income (Spouse)

A beneficiary can use this form to report the income earned outside Canada by his or her spouse if the spouse is now living or has lived outside Canada. This enables Retraite Québec to calculate the Family Allowance amount.

Person Living Outside Québec

A person who has left Québec temporarily can use this form to inform Retraite Québec of his or her situation. It can also be used by a person who believes he or she still has ties with Québec that maintain residence status. The form cannot be used to inform Retraite Québec of a change of address in Québec.

Change of beneficiary between two spouses in the same family

Family Allowance is paid to one person per family for all children that live at the same address. This form must be completed by the person who is currently receiving Family Allowance payments in his or her name, but who wants to transfer the payments in his or her spouse's name.
