Funeral expenses accepted - Application for a death benefit

Our agency will reimburse the person who paid the funeral expenses, up to a maximum of 2 500 $, for the following expenses, upon presentation of payment receipts:

  • transportation of the body
  • embalming
  • casket
  • funeral urn and niche
  • visitation at the funeral home
  • funeral service
  • burial or cremation
  • the services of a funeral director
  • publication of notices of death
  • cemetery lot (in proportion to the expenses for the deceased)
  • funeral monument (or inscription) (in proportion to the expenses for the deceased)
  • expenses incurred for telephone calls or telegrams to reach or attempt to reach members of the immediate family
  • thank you cards
  • taxes charged on reimbursable expenses
