This form allow you to cancel an application for benefits under the Québec Pension Plan (QPP): retirement pension, disability pension, pension for a disabled contributor's child, surviving spouse's pension, orphan's pension or death benefit.
If you are receiving your retirement pension under the QPP and you are age 65 or over, you may have chosen to stop contributing on your new employment earnings. In that case, if you cancel your pension before the six-month period allowed, which begins after the first payment of your retirement pension, you must repay the amounts you received and start contributing to the QPP once again. To do so, you must inform your employer as soon as possible so that the required contributions can be deducted from your salary. You may also need to pay contributions for the previous months for which you should have paid.
