It is also possible that Retraite Québec asks you to complete an application for Family Allowance for another situation.
If your child was born in Québec, you do not need to file an application because registration is carried out automatically. When you send your newborn's birth certificate to the Directeur de l'état civil, you are automatically signed up to receive Family Allowance.
This form allows you to change the frequency in Family Allowance payments. Family Allowance payments are usually made 4 times a year (in July, October, January and April), but you could choose to receive monthly payments. You will be able to revert to quarterly payments as needed.
This form allows you to inform us of any changes to your conjugal status.
The parent who is currently receiving Family Allowance payments in his or her name must use this form to inform us of a change in his or her conjugal status. In the case of a separation, you must be separated from your spouse for at least 90 days before notifying us.
The parent who is not receiving Family Allowance in his or her name must file an application for Family Allowance if the child lives with him or her at least 40% of the time each month.
If the change in conjugal status also involves a change in child custody, you must inform us immediately by telephone.

Use this online service to sign up for direct deposit of Family Allowance payments or make a change to your banking information. The application can only be used for deposits in Canada.
A person who has left Québec temporarily can use this form to inform Retraite Québec of his or her situation. It can also be used by a person who believes he or she still has ties with Québec that maintain residence status. The form cannot be used to inform Retraite Québec of a change of address in Québec.
An electronic signature is accepted on this form.
