This form allows you to file an Application for Family Allowance.
For any child born in Québec after 31 December 2006, you do not need to file an application for Family Allowance. The Directeur de l'état civil notifies Retraite Québec of all Québec births and registration for Family Allowance is made automatically.
Notice to disabled persons
A Family Allowance beneficiary can use this form to notify Retraite Québec of a change in conjugal status.
Should your conjugal situation change, the amount of your Family Allowance payment will be recalculated. To avoid having to repay overpayments, you must take the following steps according to the changes in your situation:
Change in conjugal status
The parent who is currently receiving Family Allowance payments must use the online service to inform us of the change in conjugal status.
The parent who is not receiving Family Allowance payments in his or her name must file an application for Family Allowance payments if the child or children live with him or her at least 40% of the time per month.
Change in child custody
If the change in conjugal status also involves a change in child custody, you must inform us immediately by telephone .
Use this online service to sign up for direct deposit of child assistance payments or make a change to your banking information. The application can only be used for deposits in Canada.
