Reassessment report of the various medical conditions for the eligibility to the Supplement for Handicapped Children

The reassessment reports include:

• the Parent's part, which must be completed by the parent;
• the Consent Regarding the Release of Medical, Psychosocial and Education-Related Information;
• the Professional's part, which must be completed by the health professional (physician, physiotherapist, occupational therapist, etc.) who is most familiar with the child's current condition. This person must be a member of a professional order and have assessed or seen the child again in the last 12 months.
