Application for the Supplement for Handicapped Children and Application for the Supplement for Handicapped Children Requiring Exceptional Care (Simplified version)

Important!

This form is intended for the person who is already receiving Family Allowance and want to apply for the Supplement for Handicapped Children, as well as the Supplement for Handicapped Children Requiring Exceptional Care in a single step, because he or she believes that his or her child may be entitled to both supplements, based on our eligibility requirements.

Retraite Québec must determine whether your child is eligible for the Supplement for Handicapped Children before assessing the child's eligibility for the Supplement for Handicapped Children Requiring Exceptional Care.

You are entitled to this financial assistance if:

  • you receive Family Allowance for a child
  • this child is under age 18 and has severe and multiple disabilities that prevent him or her from carrying out his or her life habits, or whose medical condition requires specified complex medical care at home. The expected duration of the disabilities or medical care must be for at least one year. We determine your child's eligibility based on certain requirements.

The simultaneous application form includes:

  • the application for the Supplement for Handicapped Children:
    • the Parent's part, which must be completed by the parent
    • the Professional's part, which must be completed by the professional (physician, physiotherapist, occupational therapist, etc.) who has assessed or treated your child and is most familiar with the child's condition
  • the Educational Achievement Report (School or Daycare)
  • the application for the Supplement for Handicapped Children Requiring Exceptional Care
  • the Consent Regarding the Release of Medical, Psychosocial and Education Related Information.
