Application for Child Assistance Payment

For any child born in Québec after 31 December 2006 , you do not need to file an application for child assistance. The Registrar of Civil Status notifies the Régie of all Québec births and registration for child assistance is made automatically.

This form allows you to file an application for child assistance if you are a new resident of Québec or in the case of an adoption, a change in custody or shared custody of a child.

A person who has left Québec temporarily can use this form to inform the Régie of his or her situation. It can also be used by a person who believes he or she still has ties with Québec that maintain residence status. The form cannot be used to inform the Régie of a change of address in Québec.
