On 1 January 2016, the Commission administrative des régimes de retraite et d’assurances and the Régie des rentes du Québec merged into one agency under the name of Retraite Québec.
We are currently modifying all of our publications to reflect the new organization. During the transition period, the former names of both organizations will appear on certain documents. However, the information you will find in the documents will not change.
Forms concerning child assistance payments

Child assistance measure
This form allows you to file an Application for Child Assistance Payments.
For any child born in Québec after 31 December 2006, you do not need to file an application for child assistance payments. The Directeur de l'état civil notifies Retraite Québec of all Québec births and registration for child assistance is made automatically.

A child assistance beneficiary can use this form to notify Retraite Québec of a change in conjugal status.

. You can file your application by completing the downloadable form in PDF format. Mail the form to us at the address shown on the form.
Use this online service to sign up for Direct Deposit or make a change to your Direct Deposit for Child assistance measure. The application can only be used for deposits in Canada.
New residents of Québec must use this form to report any income earned outside Québec or Canada for the years required in order to calculate the amount of the family allowance.
LPF-812 form. Last update: January 2016.
A beneficiary and his or her spouse can use this form to report any income earned outside Québec or Canada for the years required in order to calculate the amount of the family allowance.
LPF-807 form. Last update: January 2016.
A beneficiary can use this form to declare to Retraite Québec his or her Canadian immigrant status or that of his or her spouse if neither one is a Canadian citizen.
LPF-810 form. Last update: April 2016.
This form is for former spouses who share the child assistance payments and who wish to inform Retraite Québec that the percentage of custody time per month has changed.
LPF-809 form. Last update: January 2016.
This form must be filled out and sent to retraite Québec if a parent no longer has custody of a child following a change in custody or if the child has left the family.
LPF-806 form. Last update: January 2016.
A beneficiary can use this form to report the income earned outside Canada by his or her spouse if the spouse is now living or has lived outside Canada. This enables Retraite Québec to calculate the child assistance amount.
LPF-808 form. Last update: January 2016.
A person who has left Québec temporarily can use this form to inform Retraite Québec of his or her situation. It can also be used by a person who believes he or she still has ties with Québec that maintain residence status. The form cannot be used to inform Retraite Québec of a change of address in Québec.
LPF-811 form. Last update: January 2016.
Note that child assistance payments are made to only one person per family. Spouses can use this form to make a change to the person who receives child assistance for all the children.
LPF-802 form. Last update: January 2016.
This form allows you to authorize a designated person to obtain confidential information or documents regarding you from Retraite Québec.
