Application for Child Assistance Payment

For any child born in Québec after 31 December 2006 , you do not need to file an application for child assistance. The Registrar of Civil Status notifies the Régie of all Québec births and registration for child assistance is made automatically.

This form allows you to file an application for child assistance if you are a new resident of Québec or in the case of an adoption, a change in custody or shared custody of a child.

Change in Conjugal Status

A child assistance beneficiary can use this form to notify the Régie of a change in conjugal status.

Request for Direct Deposit or Notice of a Change

Use this form to sign up for Direct Deposit of your benefits or to notify the Régie of a change in your Direct Deposit information for child assistance. The form can only be used for deposits in Canada.

Use this form to apply for a review of a decision rendered by the Régie des rentes du Québec with respect to child assistance.

Foreign Income (new resident of Québec)

New residents of Québec must use this form to report any income earned outside Québec or Canada for the years required in order to calculate the amount of the family allowance.

Immigrant Status in Canada

A beneficiary can use this form to declare to the Régie his or her Canadian immigrant status or that of his or her spouse if neither one is a Canadian citizen.

Change in Percentage of Custody Time

This form is for former spouses who share the child assistance payments and who wish to inform the Régie that the percentage of custody time per month has changed.

Change in the Number of Dependent Children

This form must be filled out and sent to the Régie if a parent no longer has custody of a child following a change in custody or if the child has left the family.

Statement of Foreign Income (Spouse)

A beneficiary can use this form to report the income earned outside Canada by his or her spouse if the spouse is now living or has lived outside Canada. This enables the Régie to calculate the child assistance amount.

Person Living Outside Québec

A person who has left Québec temporarily can use this form to inform the Régie of his or her situation. It can also be used by a person who believes he or she still has ties with Québec that maintain residence status. The form cannot be used to inform the Régie of a change of address in Québec.

Change of Beneficiary Between Two Spouses in The Same Family

Note that child assistance payments are made to only one person per family. Spouses can use this form to make a change to the person who receives child assistance for all the children.
