Application for a Supplement for Handicapped Children

This form is to be used by the person who has a handicapped child in order to apply for a Supplement for Handicapped Children.

You are entitled to this Supplement if:

  • You care for a child who is under age 18 with a physical impairment or a mental function disability that substantially limits the child in carrying out his or her life habits for a foreseeable period of at least one year. We determine your child's eligibility according to certain criteria.
  • You are receiving Family Allowance for your child.

If you wish to apply for the Supplement for Handicapped Children and the Supplement for Handicapped Children Requiring Exceptional Care at the same time, Retraite Québec must first determine whether your child is eligible for the Supplement for Handicapped Children before assessing the child's eligibility for the Supplement for Handicapped Children Requiring Exceptional Care.

The Supplement for Handicapped Children form has two parts:

  1. One part that must be completed by one of the parents and that includes the Educational Achievement Report (School or Daycare) form.
  2. One part that must be completed by the professional (physician, physiotherapist, occupational therapist, etc.) who has assessed or treated your child and is most familiar with the child's condition.
