Use this form to apply for a review of a decision rendered by the Régie with respect to a retirement pension, surviving spouse's pension, death benefit, orphan's pension or partition of earnings.

Reçu pour frais funéraires acquittés (réservé aux entreprises funéraires)

(Receipt for funeral expenses paid - reserved for funeral homes; French only)

This form allows funeral homes to provide the Régie with a receipt proving that the funeral expenses were paid by the person who files an application for the death benefit as the person who paid the funeral expenses.
